Relocation Service Coordination-Targeted Case Management (RSC-TCM)
We provide comprehensive assistance to access essential services, including arranging transportation to visit clients for developing or implementing their care plans. Our role involves coordinating, monitoring, and supporting the overall service delivery, ensuring that services are of high quality, appropriate, and continue to meet the client’s needs. We work closely with facility discharge planners during the 180 days prior to discharge, maintaining detailed documentation to support and verify our activities. Routine communication is conducted with legal representatives, primary caregivers, clients, family members, service providers, and any other individuals necessary for the development and implementation of care goals.
Housing Stabilization Services
Housing Stabilization Services is a benefit under Medical Assistance designed to assist individuals with disabilities and seniors in finding and maintaining stable housing. These services are part of the state plan for Home and Community-Based Services (HCBS), not a waiver program.
Moving Home Minnesota (MHM)
Moving Home Minnesota (MHM) is a federal demonstration initiative aimed at facilitating the transition of Minnesotans from institutional settings to their own homes within the community.
